Sciatica

Sciatica is not a diagnosis by itself — it is a description of pain that follows the sciatic nerve from the lower back through the buttock and down the leg. Finding what is irritating the nerve is the key to relieving it.

What can irritate the sciatic nerve

  • A bulging or herniated lumbar disc pressing on a nerve root
  • Narrowing of the spinal canal or nerve openings (stenosis)
  • Sacroiliac joint dysfunction
  • Tightness of the piriformis muscle in the buttock
  • Degenerative changes in the lower spine

Symptoms

  • Pain that radiates from the lower back into one leg
  • Numbness, tingling, or a burning sensation along the leg
  • Weakness in the leg or foot
  • Pain that worsens with sitting, coughing, or sneezing

How we approach sciatica

Examination includes orthopedic and neurological testing to identify where the nerve is being irritated. Care may include adjustments to the lumbar spine and pelvis, soft-tissue work for the hip and buttock muscles, and specific stretches to reduce pressure on the nerve. Where appropriate, other in-office therapies may be discussed.

Leg weakness that is getting worse, or any change in bladder or bowel control, requires urgent evaluation.
